correct, you will be called immediately. If you do not

If your current policy is expiring, please call to confirm immediately after
sending. Your new policy/declarations
page will be emailed
within 3-5 business days.
Coverage is NOT bound until
a binder/declarations page is issued.
Instructions for Completing & Submitting
your E&O Insurance Application
1. Click on your
state above for an application. If application fails to open please contact OREP.
2. Please note the enclosed checklist to ensure a smooth process.
To avoid delays, please
type or print clearly when completing the application.
If you are
using a free version of Acrobat, please print or save the completed
application before emailing/faxing, otherwise it will arrive blank. Please
remember to sign and date the second page of the application.
3.
E-mail or fax your completed application with payment using OREP's payment page.
4. If you have current coverage in
place with a company other than OREP, please include
a copy of your expiring Declarations Page
to retain prior acts coverage.
5. Group applications and those for Louisiana and New York require quoting. All others should be submitted with
payment for same day coverage in most cases.
